「Skilled Nursing Facilities and Post-hospitalization Options for Older Adults」が掲載されている。

Abstract

Hospitalized older adults are not always able to return home immediately following discharge. Subsequently, they are discharged to facilities that provide skilled nursing, medication therapy, and rehabilitation services. Clinicians are asked by families, patients, and staff members on what is the preferred location for the older adult to be discharged to. Patients want to know what the clinicians believe to be the best option, and as a result, clinicians need to understand the options available. This chapter provides information on the post-acute setting available to older adults following a hospitalization and key features of each. Also included are resources to make the decision for post-acute care that can also be provided to family members and patients.

SNF(Skilled Nursing Facilities)については、高度看護施設と訳される。日本での考え方としては、亜急性期の施設、回復期リハビリテーション病院といった位置づけなのか。
日本では、病院、施設、在宅との連携が重要視されているため、SNFについては、あまり触れられていないが、今後入院高齢者の増加に伴い、SNFに近い施設が増加していくのか、注目していきたい。
