「Skilled Nursing Facilities and Post-hospitalization Options for Older Adults」が掲載されている。

Abstract

Hospitalized older adults are not always able to return home immediately following discharge. Subsequently, they are discharged to facilities that provide skilled nursing, medication therapy, and rehabilitation services. Clinicians are asked by families, patients, and staff members on what is the preferred location for the older adult to be discharged to. Patients want to know what the clinicians believe to be the best option, and as a result, clinicians need to understand the options available. This chapter provides information on the post-acute setting available to older adults following a hospitalization and key features of each. Also included are resources to make the decision for post-acute care that can also be provided to family members and patients.

SNF（Skilled Nursing Facilities）については、高度看護施設と訳される。日本での考え方としては、亜急性期の施設、回復期リハビリテーション病院といった位置づけなのか。
日本では、病院、施設、在宅との連携が重要視されているため、SNFについては、あまり触れられていないが、今後入院高齢者の増加に伴い、SNFに近い施設が増加していくのか、注目していきたい。

国家試験問題からの嚥下復習

摂食・嚥下リハビリテーションは医師、歯科医師、PT、OT、ST、管理栄養士（リハ栄養的アプローチ）等により実施され他職種が多く関わる。そのため、国家試験にも言語聴覚士国家試験以外でも出題されている。そのため、嚥下障害について問う問題を言語聴覚士国家試験以外から検索した。 104回歯科医師国家試験 109　摂食・嚥下障害で咽頭期に起因する症状はどれか。 2つ選べ。 a むせる。 b 咀嚼に時間がかかる。 c 食事後に声が変化する。 d 口から食物がこぼれる。 e 食渣が口腔前庭に停滞する。 解　５期モデルに関する問題でありac以外は口腔期の問題が大きいと言える。 第４４回理学療法士国家試験 午後問題６５ 脳卒中患者の摂食・嚥下障害で誤っているのはどれか。 １　急性期に高頻度にみられる ２　体位調整は誤嚥防止に役立つ ３　仮性球麻痺があると生じやすい ４　水はペーストよりも誤嚥しやすい ５　右側に咽頭麻痺では顔を左に向けて食べさせる 解　これは明らかに５であり、右の咽頭麻痺では右を向く（回旋嚥下）使用する。 他にもあるが、このように摂食・嚥下障害は言語聴覚士以外の国家試験でも出題されていることからも、多くの職種が取り組むべき障害であることを国も認識しているのではないかと思われた。

口腔アセスメント（ROAG）について

以前、OAGアセスメントについて述べたが、その改良版であるROAGの紹介である。 Andersson P, Hallberg IR, Renvert S.Spec Care Dentist.;22:181-6. 2002に　 「 Inter-rater reliability of an oral assessment guide for elderly patients residing in a rehabilitation ward .」が掲載されている。 http://onlinelibrary.wiley.com/doi/10.1111/j.1754-4505.2002.tb00268.x/pdf  要旨は The aim of this study was to test the inter-rater reliability of a revised oral assessment guide (ROAG) for patients residing in a geriatric rehabilitation ward. A consecutive sample of 140 patients was recruited for the study. Oral assessments were performed for 133 newly admitted patients by one registered nurse (RN) during a period of six months. A dental hygienist (DH) carried out 103 oral assessments during the same half-year. For 66 patients, the RN and the DH performed independent assessments. 大脳基底核と運動の関わり

Frontiers in Parkinson Diseaseに「 歩行の制御と障害のメカニズム 」 （高草木薫, 3(3) : 150-153, 2010.）が掲載されている。 　要旨は、「大脳基底核の出力は, 大脳皮質に作用することで運動の開始や精緻運動など運動の随意的側面を, そして, 脳幹に作用することにより姿勢や筋緊張, 歩行や嚥下など定型的なパターン運動を調節する. したがってパーキンソン病では, これらの運動機能の障害が出現する。 　 歩行運動には3つの側面 がある. 第一の側面 は正確な制御を必要とする随意的な歩行動作である. 歩行の開始や障害物を回避する足の動きがこれに相当する. この随意的な歩行のプロセスには大脳皮質と大脳基底核（以下, 基底核）, そして小脳から構成される認知ループと運動ループの活動が必要である. 　  第二の側面 は捕食や逃避, 逃走などの情動行動である（情動的プロセス）. 大脳辺縁系（以下, 辺縁系）や視床下部から脳幹への投射系がこれに関与する. 情動行動では, これを誘発する信号の種類にかかわらず歩行や筋緊張の亢進など定型的な運動パターンが誘発される. 　随意運動も情動行動も，歩行におけるリズミカルな四肢（手足）の運動やその際の姿勢制御には脳幹・脊髄における感覚一運動統合が必要であり，このプロセスは無意識かつ自動的に遂行される（自動的プロセス）。これが 第三の側面 であり，生得的なパターン運動（嚥下・咀嚼・サッケードなど）もこれに含まれる。」と述べている。 　嚥下機能に注目すると、大脳基底核が嚥下反射に影響を及ぼすことはすでに報告されている。また、挺舌時tremorを示す患者がいるが、大脳基底核による影響が考えられる。となると、短絡的であるが、嚥下反射とtremorの有無・程度は関連があるのかという疑問が出てくる。可能であれば、関連性を調べていきたい。
